Payment Policy

Listed below are some of the major health insurance companies which we currently accept. If your health insurance is not listed, we will be happy to verify your insurance benefits prior to treatment to ensure you will not be billed unexpectedly.

Please be aware that with any health insurance plans, there may be deductibles and co-payments for which you could be responsible. Always check with your insurance company so you are aware of payment responsibility.

Self Pay Options

We understand that insurance coverage may not always be available for our patients or we may not be contracted with your current insurance carrier.

We are happy to offer a significantly discounted self-pay rate for our patients as follows:

  • Establishing Care (Initial Visit): $180.00
  • Follow up visits: $85.00

Insurance Companies We Work With

These are just some of the many insurances we accept here at Healthy Living Family Medical. Please note that this is not a complete list of insurance companies we accept. Always check with your insurance provider to verify benefits.

Questions and Answers

Q : What about blood lab costs?  Will insurance cover that?

A :  YES.  The labs that we draw are covered by insurance. Even if we are not contracted with your insurance company such as Select Health or University of Utah Health we can run those labs.  The coverage for lab fees is between the lab and the insurance company.  They are not between Healthy Living and your insurance company. The amount they cover is dependent upon your insurance coverage and your deductible amount.

Q : Are there additional charges for Bio Meridian screening?

A : No, it is included in your office visit which would be covered by your insurance or the self pay rate.

Q : Does my insurance cover supplements or other nutraceuticals?

A : Unfortunately no.  However if you have a FLEX spending or Health Savings account, they typically will authorize those charges.  If you have any questions concerning this please give us a call.

Cancellation Policy

24 hour cancellation policy – for any scheduled appointments Healthy Living requires a 24-hour notice of cancellation or reschedule. Otherwise there will be a fee billed to the patient.   Please be sure to let us know if you need to cancel or reschedule. Thank you for your understanding.
